Add redacted evidence excerpts for the four incident cases - #130

The manuscript states that the paper repository provides redacted evidence excerpts; the directory was empty. Add one excerpt file per paper case, selected from the primary session transcripts in the escrow, plus an index: - INC-001 / Case A: the 2026-06-28 consumer verdict on w1pTime_continuous_in_H (EV-0925, session 90fa24bb) - INC-002 / Case B: the 2026-06-21 pre-merge adjudication of galerkinConvection_antisymm on PR #27 (EV-0925, session 90fa24bb) - INC-004 / Case C: the 2026-07-10 detection of the deleted public theorem edge_inf_eq_schwartz_tensor on PR #120 (EV-1669, session 7e6156bf) - INC-005 / Case D: the 2026-07-18 container memory cascade, detection and recovery (EV-0247, session de129390) Each file records the evidence ID, the JSONL line range, the incident card, the claim, and the manuscript sentences it supports. Paths containing a username or a session-specific scratch directory are replaced; Git trailers and session URLs are omitted. redact_check reports 0 findings with the denylist loaded. Freeze gate: the five new files are pinned by blob hash in scripts/check_frozen_paths.sh as owner-authorized additions (issue #127), following the one-shot exemption pattern of issues #100 and #109. […]．username を含む home directory は~，session 固有の長い作業ディレクトリは<scratchpad>に置換．Git trailer・session URL・agent 内部 ID は省いた．凍結ゲートの扱い（#90）
scripts/check_frozen_paths.shのFREEZE_EXEMPT_PINNEDに 5 ファイルを blob hash で pin した（#100・#109 と同じ one-shot 方式．issue #127 の owner 承認を根拠として script 内コメントに記載）．以後この 5 ファイルを変更すると再び gate が落ちる．.gitkeepは残した（削除は gate 上 pin できないため）．検証
denylist は #86 の対象語（local-secondary のホスト名）を含む版で実行した（issue #127 の注意書きと異なり，現 local の
private/redact-names.txtは既にこの語を含んでいた．念のため確認済み）．抜粋の選別で気づいた点（card・本文は凍結のため変更していない）
05f3967以後，マージまでの間に reviewerprrev-111-1からの独立したメッセージはメイン transcript にも同 agent の transcript にも現れない．transcript が示す修正後の確認は orchestrator 自身の grep と PR Address KSE reviewer comments on positioning, related work, and self-containedness #120 本文・merge 本文の記載である．INC-004-case-C.md末尾に境界として明記した．本文の "the reviewer repeated the comparison before merge" と card の「レビュアーが再検証を実施」は，この観察の範囲では transcript から直接には裏付けられない．oom_kill=7帰属は INC-005 の host-side 介入を再調査し、検出・回復の因果記述を訂正する #100 で否定済みのため，抜粋にその旨を付記した．Acceptance（#127）
